  • Cassia County provides this customer service as a courtesy to the public. 
  • If you must process your passport outside our established time for accepting passports, you are free to go to another passport acceptance facility.  You are not restricted by your locale.

Application Forms:

  • Estimated burden time: 10 - 15 minutes
  • Official passport application forms are available in the Recorder’s Office in the Cassia County Courthouse, Room 105
  • Online at www.travel.state.gov. Applications must be printed on separate pages. Do not print double-sided.

Applicant will need the following: 

  • DS-11 Form - Must be filled out using BLACK INK
  • Current Government issued Picture ID - NOT temporary/expired
  • Certified Birth Certificate, Expired Passport, or Original Naturalization Paper. Hospital Birth Records are NOT acceptable documents to be submitted. (The document provided will be sent in with your application but will be returned within a few days of your passport.)
  • One (1) 2”x2” Passport Photo
  • Cash or Check payable to Cassia County Recorder. The fee is $35.00 per application
  • Personal Check, Cashier’s Check or Money Order payable to Department of State – Each application must have its own check or money order – See fees below

Passport Book – Valid for travel by air, sea or land  

  • $130.00 (Age 16 and Older) {valid for 10 years}
  • $100.00 (Age 15 and Under) {valid for 5 years}



Passport Card – Valid for land & sea, crossing between the US & Canada, Mexico, Bermuda & the Caribbean ONLY  

  • $30.00 (Age 16 and Older) {valid for 10 years}
  • $15.00 (Age 15 and Under) {valid for 5 years}

Important Information  

  • Applicants must appear in person. If applicant is under 16 years of age, BOTH PARENTS & CHILDREN must be present and provide proper photo ID to sign for child. If applicant is 16 or 17, one parent must be present with proper photo ID and sign with applicant.
  • Departure dates less than 4 weeks, applicant(s) will be required to pay the additional $60.00 Expedite Fee. 
  • Additional Priority Express Delivery options are available:
    •  Priority Express Delivery to the Passport Facility is $32.00 payable to the Recorder's Office; 
    • Priority Express Return* is $21.36 payable to the Department of State.
      *Priority Express Return is not available for Passport Cards.
